Walk-In Flu Clinic

Community Health Castleton is hosting a flu vaccine clinic for students, faculty, staff, and community members. No reservation is required. 

Please print and complete the attached forms and bring them with you to the flu clinic:

• CHCRR Patient Registration form- Please complete the form and include a copy of your insurance card with the registration.  If you are not the subscriber on the insurance, please make sure to include the subscriber’s name and date of birth.
• Flu Clinic Administration form- Please complete with your name and date of birth and answer the screening questions.
