This form should be used by any person to report an alleged violation of the Vermont State Colleges policies regarding discrimination, harassment, and sexual misconduct. This form can be utilized by anyone wishing to file a report on their own behalf or on behalf of someone else.
This form is received and reviewed by the Director of Public Safety who will then forward it to the appropriate University personnel. Information submitted on this form outside of normal business hours (Monday-Friday, 8 a.m. to 4:30 p.m.) may not be reviewed until the next business day.
If there is an immediate risk to health or safety please contact Public Safety at 802-468-1215 or Castleton Police at 802-468-5012.
